    Since you have been a sincere employees,you should not lie.You should say the facts about not completing the assignment in time.You can say that due to some health problem,you could not complete the works in time.you need some more days to complete the works.

    Also you should examine why you could fail to complete your work in time.Because,being first time ,your boss may not mind,but if it repeats in future,your career may be affected to great extent.You should sort out the problems of not completing the work in time for future sake.

    I can understand your situation that you are hesitating to inform about it to your boss.But I will suggest you to imform as soon as possible.Since this is the first time,I think he may excuse you.Apologise to him for being unable to finish on time.If you inform him as soon as possible,then he would be able to handle the rest of the unfinished work himself or give the responsibility to any other person.

    As you have mentioned you have been a punctual performer till now and this is the only project where you could not complete it in time,i dont think for only this issue your boss will be mad at you and take any action against you.just approach boss and tell me under what circumstance work got delayed and ask for extension,i hope he will consider as it is your first mistake.talk politely and ask for extension and if he disagrees then ask him you will manage it in a couple of days if required you will do over time.
    and promise to keep track of the days and try to complete project in time in upcoming Projects ,he will definitely understand and support you by extening the time period.
